Wherever the full text is available online, I start the reference with an asterisk (*) and link the title. Links without an asterisk show where else to get the text in question. So far I count four books, nineteen articles, and sixty reviews with digitized text online.

Most of the reviews and several of the articles require a subscription (to de Gruyter, JSTOR, Cambridge Journals Online, etc). Universities tend to have at least JSTOR. Interpretation has offered a 7-day trial with full access, and they have images of all issues back to their first in 1947.
